What is a Support Coordinator?

A support coordinator is someone who can assist you on your NDIS journey.

LotUs Assist has Support Coordinators with various skills and backgrounds in disability to suit a wide range of needs. LotUs Assist Support Coordinators have the knowledge and the networks within both the NDIS and mainstream support to assist you to make your NDIS goals achievable.

Our Support Coordinators will work alongside you to ensure that you get the most out of your NDIS plan. We listen and help you understand how to implement your plan in a way that meets your needs and desires and link you to informal, mainstream & community support that will be the right fit. We also help build your ability to exercise choice and control, to coordinate support and access your local community. Our Support Coordinators will focus on building skills which includes working with you to find and connect to mainstream services such as employment, government departments, social supports and advocacy.

If you’re into sport, art, music, exercise, volunteering, knitting, fishing, photography or anything else our Support Coordinators will help you find the most appropriate service.

Our Support Coordinators will assist with:

  • Building your confidence and your support network
  • Setting up your MyGov account, connecting to the NDIS Participant Portal and navigating the NDIS portal.
  • Establishing Service Agreements and Service Bookings with your chosen Service Providers.
We are an independent service which provides impartial advice and focuses on your needs and your needs only.

Step 1

Our Support Coordinators work alongside you to understand your needs and desires and identify your goals so we can build the best support plan for you.

Step 2

Our Support Coordinators contact a range of appropriate service providers to gauge their availability, discuss your options and arrange a meeting for you to meet with them to allow you to make the right choice. This allows you to have control of your support, and organise agreements, services and prices to ensure that you are getting the most out of your budget.

Step 3

At LotUs Assist, our Support Coordinators will provide ongoing support for you throughout your NDIS plan to ensure that everything is running smoothly. When your plan is up for review, we can assist with that too. We will coordinate reports from your support providers, including therapists, to ensure the NDIS have the evidence required to provide reasonable and necessary funding for your next plan.

 

How a LotUs Assist support coordinator will assist you
